Eye Surgeons NZ is committed to protecting your privacy and complying with the New Zealand Privacy Act 2020, the Health Information Privacy Code 2020, and all applicable health information legislation.
Eye Surgeons is a private ophthalmology practice operating clinics in Hamilton, Cambridge, Whanganui, Pukekohe, and Thames. Our patient records are managed through CatTrax, a certified health information management system. CatTrax's privacy practices are described at cattrax.co.nz.
Clinical information: Medical history, diagnoses, treatment plans, surgical records, and visual assessments collected during consultations and procedures.
Contact and identification information: Name, date of birth, address, phone number, email address, and NHI number.
Insurance and financial information: Health insurance details, ACC claim numbers, and payment information.
Communication records: Inbound and outbound phone calls may be recorded and AI-transcribed by our phone system provider for quality assurance and service improvement. Recordings are not used for marketing purposes.
Website data: Standard analytics (pages visited, browser type) when you use our website or submit a contact form. No personally identifiable information is collected via analytics.
To provide specialist eye care and manage your treatment pathway.
To communicate about appointments, test results, and follow-up care via phone, SMS, and email.
To send appointment reminders via our patient management system (CatTrax).
To analyse call patterns and improve our phone service (call recordings are not linked to individual patient clinical records).
To process insurance claims and billing.
To comply with our legal and professional obligations as a registered medical practice.
We share your health information only where necessary for your care:
Your GP and referring practitioners: Consultation letters and surgical reports as part of coordinated care.
Health insurers: With your consent, to process pre-authorisation and claims.
Our clinical software providers: CatTrax (patient management) processes data on our behalf under confidentiality agreements. CatTrax operates within New Zealand. Our phone system provider also processes call data on our behalf; their data processing terms will be linked here once confirmed.
ACC: Required claim information for ACC-funded treatment.
We do not sell, rent, or share your personal information with any third party for marketing purposes.
Calls to and from Eye Surgeons may be recorded and processed by an AI transcription service. Transcripts are used internally for quality assurance, staff training, and service improvement. Transcripts are not stored in your clinical record. Call recordings are retained in accordance with our phone system provider's retention policy. If you do not wish your call to be recorded, please inform the staff member at the beginning of the call.
Clinical records are retained for a minimum of 10 years from the date of last treatment (Health (Retention of Health Information) Regulations 1996). Phone recordings are retained for up to 12 months. Website enquiry forms are retained for 24 months or until resolved.
